Grants and Performance Manager
Requires a Masters Degree in public health, social services or related field plus three years  management experience. Experience in writing federal grants and managing quality assurance and improvement efforts.  Comfortable with providing coaching and feedback to staff and peers.  Must be detail oriented and organized.

Quality Assurance Coordinator
Requires a Bachelor’s Degree in public health or social services or a related field plus one year related work experience.  Experience managing databases, compiling and analyzing data, and providing feedback to staff and peers on performance.   Must be detail oriented and organized.

Case Manager
Requires a Bachelor’s Degree in public health or social services or a related field plus one year related work experience.  Experience in providing case management services to diverse populations.  Willing to provide home visits to families in high risk neighborhoods.

Grants and Performance Manager      

Minimum Qualifications: A Master’s degree in public health, nursing, social work or related field. At least 3 years experience working with grant writing, quality assurance , and evaluation. A strong understanding of public health programs or social services case management. Strong communication and analytic skills. Competence in working with diverse populations. Previous supervision experience. Strong organizational and project management abilities.

DUTIES AND RESPONSIBILITIES

Program Performance

  1. Ensure accurate and consistent data collection for all departments by working with the QA/QI coordinator to provide oversight to the ChallengerSoft database.
  2. Ensure integrity of program data and program performance by overseeing internal quality assurance audits, creating executive summaries of said audits and making recommendations for quality improvement strategies.
  3. Ensure quality customer service by conducting regular participant and community partner satisfaction surveys and/or focus groups.
  4. Supervises Quality Assurance Coordinator and Data Entry.
  5. Coordinate evaluative efforts with Healthy Start New Orleans’ local evaluator as needed.
  6. Support department managers in the developing and monitoring measurable objectives for staff and their departments.
  7. Provide monthly summary reports on performance to the Program Director.

Grant Writing

  1. Researches grant opportunities to secure resources for Healthy Start New Orleans.
  2. Works with Program Director to compile and process evaluation data for all grants, and prepare timely program reports for internal and external audiences.
  3. Work with the Program Director to oversee the grant writing proposal process; ensure all deliverables (as outlined in grants) are being met, including program planning, implementation, evaluations, and reporting deliverables;ensures compliance with applicable patient and/ or state laws, regulations, and/ or agency rules, state laws and guidelines.

Other Duties

  1. Work with the Program Director to assist with the development of cooperative agreements between Healthy Start New Orleans and other community based partner organizations, as related to programming and evaluation.
  2. Develop and coordinate plans and policies, resources and missions, as well as goals,  vision and expectation of Healthy Start New Orleans. Attend relevant trainings, workshops, and conferences to stay current on programmatic and organizational priorities.
  3. Manages client and employee complaints,ensuringissues are resolved in a timely manner.
  4. Participates in staff meetings.
  5. Provides excellent customer service for both internal and external customers.
  6. Other Duties as assigned.

 

Quality Assurance Coordinator

Objective: To provide a continuous, overall Quality Assurance and Improvement strategies to ensure the Healthy Start New Orleans Program is systematically and intentionally increasing positive outcomes for its clients.

DUTIES AND RESPONSIBILITIES

  1.  Promote quality achievement and performance improvement throughout the Healthy Start Program in accordance with HRSA program measures
  2. Set Quality Assurance compliance objectives and protocol in conjunction with operating staff
  3. Assist various program managers in monitoring program achievement of mandated performance measures on a continuous basis
  4. Develop and implement a User Guide Manual for the operation of the internal ChallengerSoft database system
  5. Develop and implement auditing tools utilizing the program’s database to measure program progress and complete outstanding performance issues
  6. Set up and maintain auditing and quality controls and documentation in conjunction with the program’s Data Entry Coordinator
  7. Collaborates with the HSNO Management Team and spearhead the development of policies and procedures integral to the      operations of HSNO and in compliance with federal, state and city regulations
  8. Identifies relevant Quality Assurance training needs for direct care staff
  9. Acts as a liaison to the program managers and staff to ensure Quality Assurance system, ensuring the proper functionality of the ChallengerSoft database. When appropriate, the Quality Assurance Coordinator will also be expected to advise program managers and staff on database changes; provide trainings; and develop tools and appropriate techniques designed to enable all staff to provide high quality services
  10. Assists the Healthy Start Program Consultants in obtaining data, including the completion and writing of program reports as needed
  11. Monitors specific program activities by preparing qualitative and quantitative reports for program management in a timely manner
  12. Identifies channels to improve internal and external customer satisfaction
  13. Maintains working knowledge of applicable rules, regulations, policies, laws, and guidelines that impact the Public Health arena and the Healthy Start New Orleans program.
  14. Provides excellent customer service for both internal and external customers
  15. Maintains knowledge of current computer programs, and utilization of spreadsheet software
  16. Additional duties and responsibilities, as necessary

 

Case Manager   

DUTIES AND RESPONSIBILITIES:

  1. Conduct client home visits, prenatal, postpartum, mental health and/or newborn assessments for well clients as well as perform community outreach activities within the community as needed.
  2. Complete client intake procedures, data entry and documentation of case management services.
  3. Case managers provide one on one parenting, prenatal education teach and / or serve as staff support for HSNO Support Groups and HSNO Education Classes as well as facilitate mental health and other referrals for services and resources as needed.
  4. Refer clients to and advocate for clients to ensure they receive appropriate benefits and/or services .
  5. Facilitate client entry and linkage to various professional services.
  6. Attend bi-weekly case conference and staff meetings, team meetings, educational training sessions and health fairs as needed.
  7. Ensure all necessary information is thorough and accurate, all documents and evaluations are placed in both paper files and entered into the HSNO database system within specified timeframes.
  8. Develop or assists in the development of additional community resources as indicated by the needs of the clients.
  9. Schedule and completes contact with clients, coordinate services and conduct consistent reassessment activities or when indicated by clients’ needs or lack of progress in achieving desired outcomes.
  10. Facilitate discharge and/or transition from service systems and assigned client levels.
  11. Provide weekly activity reports describing job related activities as they pertain to the HRSA data performance indicators generated from the HSNO database.
  12. Provide referrals for nutritional supplements, contraceptives, pre/post natal care, personal care, and baby items on an as needed or incentive basis .
  13. Maintain knowledge of application rules, regulations, policies, laws and guidelines that impact personnel and administrative issues for the city and HSNO; develop internal controls that promote adherence to applicable state/federal laws, and program requirements; adhere to their licensure’s code of ethics (if applicable) and seek advice and guidance as necessary to ensure proper understanding.
  14. Provide excellent customer service for both internal and external customers; approach all encounters with a warm, professional greeting and assists to problem solve whenever necessary; and recognizes when an issue needs further attention from a supervisor and inform management so that the problems can be addressed in a timely manner.
  15. Ideal candidate should have experience in maternal and child health and possess a valid license in social work, counseling, or closely related field, or eligible to attain certification and or licensure within the first year of full time employment with   Healthy Start New Orleans.
  16. Additional duties as needed.
