Community Network Coordinator

PROGRAM: Accion Por La Salud/ADAP

REPORTS TO: Director of Access to Care Services

PRIMARY RESPONSIBILITIES

  • Develop and maintain community relations with HIV providers and Community Base organization for the referrals of clients.
  • Participate in community outreach events to provide information and to link possible client to Accion: Health fairs, Community festivals, AIDS Walk, LBGT events, parades, conferences and community based organizations special events.
  • Participate in Tabling and the distribution of program promotional materials at community events.
  • Identify Referrals for enrollments into ADAP, other healthcare coverage and supportive services
  • Support Community Health Coordinator in the screening and assessment of clients (Determination of eligibility into a healthcare coverage program)
  • Support in providing presentations on program services
  • Enter weekly data in URS and prepare outreach activity report

ADDITIONAL RESPONSIBILITIES:

Ø Data entry

Ø Enter and maintain all required information in appropriate log(s)

Ø Other Duties as assigned by Program Director

QUALIFICATIONS:

  • Bachelor’s degree in Social Work, Psychology, Community Health or related field with at least four (4) years experience in HIV counseling and testing.
  • Four (4) years Outreach experience in communities of color
  • Knowledge of Oraquick and Orasure HIV testing procedures
  • Excellent communication skills, writing and verbal
  • Computer literate Microsoft Word and Excel
  • Bilingual (English and Spanish)
